Take the "What Type of Doula Am I" survey below or use the flowchart to determine which enrollment toolkit to use.

Select the appropriate color-coded toolkit below (based on the type of doula you are) and fill out the Information form to download.

Step 3:

Fill out the enrollment forms using the PDF Toolkit as a guide.

 

 

Step 4:

Gather all application materials and mail to MassHealth
